Millia's Zorg attaches great importance to protecting the privacy of its clients, employees, and website visitors.
We treat all personal data confidentially and process it in accordance with the General Data Protection Regulation (GDPR) and the Supplementary Provisions for the Processing of Personal Data in Healthcare Act (Wabvpz) .

3. Personal data that we process
Millia's Zorg processes personal data because clients use our services.
Depending on the nature of the care, we may process the following data:
General information:

Name, address and place of residence (NAW data)

Phone number and email address

Date of birth

Citizen Service Number (BSN, if legally required)
Healthcare-related data (special personal data):

Medical and/or healthcare-related data

Information about health, care, medication or limitations

Information provided by doctors, caregivers or agencies
Financial data:

Administrative data for PGB, WLZ or WMO payments
4. Purposes of data processing
Millia's Zorg processes personal data exclusively for:

Execution of the health care agreement;

Communication with clients, relatives or referrers;

Administration, invoicing and declaration at municipalities, healthcare offices or PGB managers;

Legal obligations (such as Wkkgz, WMO, WLZ and tax legislation);
-
Internal quality improvement and accountability.
Millia's Zorg does not use personal data for commercial purposes.
5. Basis for processing
We process personal data on the basis of:

Client consent;

Execution of an agreement;

Legal obligation;

Protection of the client's vital interests;

Legitimate interest (such as quality control).
6. Retention periods
Millia's Zorg does not store personal data longer than necessary:

Care file: 15 years after termination of care (in accordance with Wabvpz);

Financial administration: 7 years (according to tax legislation);
-
Application data: maximum 4 weeks, or 1 year with permission.
After processing, data will be securely destroyed or anonymised.
7. Sharing data with third parties
Millia's Zorg shares data only when necessary for healthcare provision or when legally required, for example with:

General practitioners, district nurses or other healthcare providers;

Municipalities, healthcare offices or PGB managers;

Administrative offices or software suppliers.
We conclude a processing agreement with all external parties to guarantee the security and confidentiality of data.
8. Security of personal data
Millia's Zorg takes appropriate measures to protect personal data against loss or unlawful processing, including:

Strong password security and encryption (SSL);

Secure storage and limited access to client files;

Confidentiality obligation for all employees;

Periodic check of security measures.
9. Client rights
Every client has the right to:

to access his or her data;

to request correction of incorrect data;

request removal (to the extent legally permitted);

to object to processing;

request data transfer to another healthcare provider.

You will receive a response within four weeks .
10. Minors and representatives
For clients under the age of 16 or clients who cannot make independent decisions, personal data will only be processed with the permission of a parent, guardian or legal representative.
11. Data leaks
In the event of a suspected data breach, Millia's Zorg will immediately report this to the Dutch Data Protection Authority and, if necessary, to the client(s) involved.
12. Cookies and website
Our website only uses functional and analytical cookies that do not infringe on your privacy.
We do not place marketing cookies.
